DEEP Training Evaluations

All individuals who complete the DEEP Facilitator Training must submit a **Training Evaluation**.
This evaluation collects feedback on training content, facilitator performance, curriculum clarity, and overall readiness to deliver DEEP workshops.

Purpose:

* Ensures training quality and consistency
* Helps UIC OTM maintain program fidelity
* Identifies future training needs and improvements

Community Workshop Evaluations

After you deliver a DEEP community workshop, participants must complete the **DEEP Community Workshop Evaluation**.

Purpose:

* Measures participant satisfaction and perceived value
* Tracks relevance and usefulness of topics covered
* Helps facilitators and program administrators identify areas for improvement

Pre/Post Tests

Pre- and post-tests are critical tools for assessing participant learning and the impact of the workshop.

* Pre-Test - Completed on the first day of the workshop to measure baseline knowledge.

* Post-Test - Completed on the final day to measure new knowledge gained during DEEP.

Purpose:

* Quantifies participant learning
* Demonstrates program effectiveness
* Supports data-driven reporting for funders and partners

Facilitator Responsibilities:

* Administer both tests
* Score or record results as required
* Submit results as part of your workshop report

Facilitator Recertification

Recertification Requirements Typically Include:

* Proof of at least one completed DEEP workshop during the certification cycle
* Submission of workshop evaluations and pre/post-test data
* Updated contact and organizational information
* A short facilitator skills refresher or updated training module (if applicable)

Purpose:

* Ensures facilitators stay current with DEEP curriculum updates
* Maintains program quality, accuracy, and fidelity

Annual Reporting Requirements

Organizations and facilitators must complete Annual Reports to remain in compliance with DEEP licensing requirements.

Annual Report Includes:

* Number of DEEP workshops delivered
* Number of participants served
* Verifying active certified educators by facilitator level
